Intake assessment is conducted by asking the new client a series of targeted questions that will assist KCI on the type of assistance and/or resource connection(s) needed.
Individual action plan development acts as a step-by-step outline and road map to assist the client in determining what resources and/or assistance are required to achieve desired goal. A designated support specialist is assigned to the client to guide the process which involves "what we're going to do" and "how we will go about doing it."
KCI maintains an extensive resource and information database of available community resources and other supportive services available for staff to access needed information to make appropriate referrals and resource connections.
Involves the individual independently speaking on their behalf to the community agencies and (KCI) will be available to provide assistance if needed.
Involves KCI staff/volunteers initiating activities to make changes in that community that makes it easier for our clients to access community services.
KCI provides workshops, training modules and videos that address the needs of the community and enhances educational awareness and self-sufficiency.
